All students interested in trying out for a sport for HCPSS MUST have on file with the Coach, or bring with them on to tryouts, the following:
1. Pre-Participation Physical Examination (dated on/after April 1, 2008)
2. Parent Permission Form
3. Photo Copy of their Birth Certificate (only if this is their first time participating in a sport at MTH)
4. A Photocopy of their 4th Quarter Report Card (applies only to 10, 11 & 12th Graders)
5. Residency Verification (in the form of a BGE Bill, Water Bill or Land Line Phone Bill)
